Choices Person-Centered Services

Choices Person-Centered Services (formerly known as Choices Transitional Services) provides people who have disabilities the opportunity to uphold individual dignity and promote a healthy, independent lifestyle.

Choices Person-Centered Services was established in 1988 and has been providing progressive programs and services to adults with developmental disabilities for over 25 years. The specialized programs offered by Choices promote independence, personal development, self-sufficiency and respect for the adults it serves. Choices offers its clientele three very different programs by which to tailor their individual life path: Supported Living Services, Independent Living Services, and Adult Day Program Services.
